Key Responsibilities:- Establish connections between patients and relevant services, utilizing a comprehensive network of contacts in both correctional and community settings.
- Collaborate closely with healthcare providers to ensure optimal patient outcomes and continuity of care.
- Experience in managing complex health needs for clients with chronic conditions.
- Understanding of drug and alcohol treatment approaches and experience working with community-based networks.
- Qualifications include allied health disciplines such as Social Work, Counselling, Occupational Therapy or Psychology.
