Job Description

Hearth is a NDIS Registered Service Provider and we have an exciting new opportunity for a passionate & enthusiastic Relationship Manager to join our friendly team in Bendigo.

We aim to create an environment that nurtures, supports, and engages with our participants and support workers to consistently provide the highest standard of care.

About the role

This Relationship Manager will be the central point of contact for support workers and participants.

The successful candidate will ensure that participants, their family members, support workers, and all other stakeholders have a clear & consistent point of contact.

Key Responsibilities
  • Create and continuously develop relationships between participants and their team of support workers.
  • Assess participant needs, and develop Hearth's support program.
  • Maintain relationships with support coordinators, house supervisors, and other relevant external stakeholders.
  • Lead the Support Worker teams to provide responsive, person-centered care that enables participants to progress toward their goals.
  • Report on the progress of clients, as it relates to the support provided by Hearth, and prepare submissions for NDIS plan reviews as required.
  • Coordinate a weekly staff roster for each participant and proactively resolve rostering issues.
  • Audit case notes and client reviews to reflect forward planning in line with the participant's NDIS plan.
  • Ensure compliance with policies and procedures to maintain a safe working environment and timely incident reporting.
Requirements

The ideal candidate will have:

  • Proven experience working in the disability sector, preferably in a supervisory or managerial role.
  • A professional level of administrative and business communication skills, including case noting and report writing.
  • Excellent time management and organisational skills.
  • Client centred approach with the ability to advocate for participants and an understanding of complex disability and trauma.
  • Own car and driver's license.
  • Strong interpersonal skills and high levels of resilience.
  • Understanding of the NDIS.
  • Working knowledge of Microsoft Office suite and employee rostering systems.
