
Care Coordinator

This role is a pivotal part of our commitment to delivering exceptional care and support to individuals struggling with alcohol and other drug harms, mental health challenges, and social disadvantage.
\Key Responsibilities\
- \
- Lead and coordinate the day-to-day operations of the Nursing team at our facility.\
- Mentor and develop a small team of Enrolled Nurses through peer consultation and training.\
- Conduct thorough nursing risk assessments and monitor residents' pre-existing medical conditions throughout their stay.\
- Participate in admissions, intake, and discharge reviews, ensuring comprehensive consideration of Physical Health and Mental Health as Life Domains.\
- Collaborate with General Practitioners and allied health professionals to provide holistic care and support.\
- Administer medications according to prescriptions and orders, observing clinical features and ensuring desired effects.\
- Document client information accurately and comprehensively, capturing relevant details for analysis.\
- Evaluate treatment responses and report new information, changes in condition, or concerns regarding vital signs and withdrawal symptoms to the treating doctor or registered nurse.\
Requirements\
We are seeking a Registered Nurse with:
\- \
- 3+ years' experience working within the alcohol and drug sector, mental health, or primary healthcare environment, with an understanding of AOD service navigation and complex needs.\
- Demonstrated expertise in conducting drug and alcohol withdrawal and physical assessments.\
- Knowledge of contemporary standards and evidence-based practices in AOD contexts.\
- Exceptional interpersonal skills and natural ability to communicate effectively with diverse groups.\
- Bachelor's degree in nursing and current registration as a Registered Nurse (Division 1) within the Australian Health Practitioner Regulation Agency (AHPRA).\
- Ideal certification in AOD or willingness to complete an AOD Skillset.\
