CKD Nurse Practitioner Role Overview

The CKD Nurse Practitioner plays a critical role in supporting patients with end-stage renal disease. This professional streamlines assessments, referrals, and ongoing reviews across the care continuum.

Key responsibilities include managing stage 4/5 CKD cases, delivering tailored education, and supporting informed decision-making. The role also develops Renal Supportive Care services for patients choosing conservative care or nearing end of life.

This position requires a strong understanding of renal care, with expertise in treatment decisions, patient education, and supportive care. The successful candidate will have excellent communication and interpersonal skills, enabling them to work effectively with patients, families, and healthcare teams.

  • Current Registration as a Registered Nurse with Nursing and Midwifery Board of Australia via AHPRA.
  • Minimum of 5 years experience in Renal nursing.
  • Post graduate Masters of Nurse Practitioner qualification.
  • Motivated, autonomous with an ability to work collaboratively with departments across many health services.
  • Proven clinical leadership skills and a commitment to quality and safety.
