Our Family Based Care team is seeking a skilled Case Manager to collaborate closely with children, young people, their families and carers. The successful candidate will work towards creating and implementing case plans that achieve permanency goals.
About the Role- Develop strong relationships with children and their families
- Collaborate with multidisciplinary teams to create effective case plans
- Support and monitor placements, guiding carers in their foster care journey
- Bachelor's degree in Social Work, Social Welfare or Psychology
- Previous experience in Case Management within Out of Home Care /Child Protection settings
- Understanding of trauma and its effects on children and young people
- A supportive team environment
- Ongoing training and professional development opportunities
