Therapeutic Care Specialist

We are seeking a highly skilled Therapeutic Care Specialist to join our multidisciplinary team. In this critical role, you will play a pivotal part in delivering comprehensive aged care assessments that cater to the diverse needs of our clients.
Key Responsibilities:- Conduct thorough and efficient aged care assessments in accordance with Department of Health and Aging guidelines and ACAT protocols.
- Provide informed support and guidance to patients and their families, empowering them to make informed decisions about their care.
- Foster effective collaboration with healthcare professionals to ensure seamless assessment processes for clients in both acute care facilities and community settings.
- Promote equitable access to Australian government-funded aged care services, adhering to uniform standards and best practices.
- Holding current AHPRA registration is essential for this role.
- Aged Care Check certification is also mandatory.
- Vaccination requirements must be met for Category A roles.
- A valid NSW driver's license is required.
