Support Worker Job Description

We are seeking a Support Worker to join our team and deliver personalized care in line with individual care plans.

As a Support Worker, you will be responsible for providing domestic and personal assistance to elderly individuals, including housekeeping, meal preparation, shopping, appointment management, and assistance with banking and bills. You will also support clients with hygiene, mobility, medication management, continence care, and more for both low and high-needs clients.

Key Responsibilities:
  • Domestic & Personal Assistance: Provide daily living support, including housekeeping, meal preparation, and shopping.
  • Personal Care: Assist with hygiene, mobility, medication management, and continence care for elderly individuals.
  • Rehabilitation & Independent Living: Support rehabilitation activities, therapy, and transportation as needed.
  • Care Planning & Reporting: Ensure accurate care plan delivery, report changes, and maintain strong relationships with clients, families, and healthcare professionals.
Required Skills and Qualifications:

To be successful in this role, you will require:

  • Experience in personal care (low and high needs).
  • Strong communication skills and ability to work with elderly clients.
  • A compassionate and respectful approach to elderly individuals.
  • The ability to manage tasks efficiently and work independently.
