**Clinical Lead - Registered Nurse**

We are seeking a highly skilled and experienced Clinical Lead - Registered Nurse to join our team at Help at Hand Support Services. As a key member of our leadership team, you will play a pivotal role in overseeing the clinical operations and care coordination within our organization.

**Key Responsibilities:**

  • **Leadership and Management:**
    • Lead and manage a team of Care Coordinators, support workers, and other stakeholders.
    • Develop and implement clinical policies, procedures, and best practices to ensure high-quality care and compliance with NDIS standards.
    • Provide clinical supervision, mentorship, and support to staff to enhance their skills and professional growth.
    • Conduct regular performance evaluations and provide constructive feedback to team members.
  • **Care Coordination:**
    • Oversee the coordination and delivery of care services to clients, ensuring their needs are met in a timely and effective manner.
    • Collaborate with clients, their families, and the care team to develop and review individualized care plans.
    • Ensure seamless communication and collaboration between all stakeholders involved in a client's care.
    • Monitor and evaluate the effectiveness of care plans and make necessary adjustments to improve outcomes.
  • **Clinical Excellence:**
    • Maintain up-to-date knowledge of best practices in nursing and disability care.
    • Conduct regular audits and quality assurance checks to ensure compliance with clinical standards and NDIS requirements.
    • Address and resolve clinical issues and complaints in a timely and professional manner.
    • Promote a culture of continuous improvement and innovation in clinical practices.
  • **Compliance and Reporting:**
    • Ensure all clinical documentation is accurate, complete, and compliant with regulatory requirements.
    • Prepare and submit reports on clinical activities, outcomes, and key performance indicators to the management team.
    • Stay informed about changes in NDIS policies and regulations and ensure the organization's practices are aligned.

**Qualifications and Experience:**

  • Bachelor's degree in Nursing (required)
  • Current registration with the Australian Health Practitioner Regulation Agency (AHPRA)
  • Minimum of 2-3 years of experience in a clinical leadership role within the disability or healthcare sector
  • Comprehensive understanding of the NDIS framework and experience in care coordination within the NDIS space
  • Strong leadership and management skills with the ability to inspire and motivate a diverse team
  • Excellent communication, interpersonal, and problem-solving skills
  • Demonstrated commitment to person-centered care and continuous improvement
