Empowering lives through personalized support is at the heart of this meaningful role. As an NDIS Support Coordinator, you will work closely with participants to understand their needs and goals, connect them with relevant services and break down barriers.
About The PositionThis is a unique opportunity for someone motivated, organized and passionate about making a difference in people's lives. You will be responsible for building strong relationships with participants, their families and support providers, helping them understand and use their NDIS plans, coordinating supports and negotiating service options.
Key Responsibilities:- Establish trust-based relationships with participants, their loved ones and support networks
- Facilitate participant understanding and utilization of their NDIS plans
- Coordinate tailored support services and negotiate optimal service arrangements
- Engage in regular face-to-face interactions with participants across local areas
- Balance time between participant-centered work and meeting performance targets
- Relevant experience in disability, community services or similar fields
- Excellent administration and organizational skills
- Confidence in working with digital tools and systems, particularly Excel
- Ability to establish rapport, communicate effectively and support individuals from diverse backgrounds
- A proactive, adaptable mindset and a drive to learn and grow
