This role is ideal for an experienced professional seeking to make a positive impact in the lives of individuals with disabilities.
We are currently seeking a skilled Support Worker to join our team, where you will have the opportunity to deliver high-quality support services and make a real difference in the community.
As a Support Worker, you will be responsible for providing personalized support to customers, enabling them to achieve their life goals and improve their overall well-being.
You will work collaboratively with our customers, their families, and other stakeholders to develop and implement individualized support plans that promote independence, dignity, and choice.
Key Responsibilities- Deliver person-centred support to customers in their homes or community settings, promoting independence and empowering them to reach their full potential.
- Develop and maintain effective relationships with customers, their families, and other stakeholders, ensuring that their needs and preferences are understood and respected.
- Collaborate with multidisciplinary teams to identify customer needs and develop comprehensive support plans that address their physical, emotional, and social requirements.
- Conduct regular assessments to evaluate customer progress and adjust support plans accordingly, ensuring that they remain relevant and effective.
- Maintain accurate records of customer interactions, support plans, and outcomes, utilizing data to inform service improvements and quality assurance.
- Relevant qualifications (Cert III in Disability, Individual Support, or higher) and extensive experience in supporting customers with complex needs.
- Strong communication and interpersonal skills, with the ability to build rapport with customers, their families, and other stakeholders.
- Ability to work independently and as part of a team, prioritizing tasks and managing multiple responsibilities effectively.
- Valid driver's license and willingness to use your own vehicle, subject to Activ's motor vehicle allowance rebate.
