About the RoleWe seek an exceptional individual to join our team as a Clinical Aged Care Assessor. This is an opportunity to make a meaningful impact in the lives of older adults by identifying their needs and connecting them with essential services.
In this role, you will conduct thorough assessments to understand the needs of older adults, facilitate referrals to relevant services, promote functionality, engage with stakeholders, and ensure compliance with Department of Health contractual requirements.
Your Key Responsibilities- High-Quality Assessments: Conduct detailed aged care assessments to identify the needs of older adults, taking into account their unique circumstances and requirements.
- Service Referrals: Facilitate smooth transitions by referring clients to appropriate services that support independent living, enhancing their overall quality of life.
- Promote Functionality: Actively promote the retention and maximization of function and capability in daily living activities, empowering older adults to maintain their independence.
- Engage Stakeholders: Build strong relationships with families and carers, recognizing their importance in the client's journey.
- Compliance: Ensure all assessments are conducted in accordance with Department of Health contractual requirements, upholding the highest standards of professionalism and integrity.

What You Need- A minimum of one year's experience in aged care, including client assessments and/or service delivery.
- Tertiary education in a health-related discipline, such as nursing, occupational therapy, physiotherapy, or social work.
- Current unrestricted General Registration with AHPRA or relevant professional association.
- Proficiency in Microsoft Office applications, including Word, Excel, and Outlook.
- A current Australian driver's license, comprehensively insured vehicle, and the ability to travel within your designated service area.
