We are seeking a motivated and experienced professional to fill a dynamic role as a Service Facilitator. As a key member of our team, you will be responsible for delivering high-quality support services to clients in the community and in their homes.

As a Service Facilitator, you will have the opportunity to work collaboratively with staff across teams to ensure the best possible customer service level is achieved. Your daily management of rosters will ensure accuracy and timely claims for payment.

Key Responsibilities:

  • Implement and maintain individual client goals through high-quality support services.
  • Manage rosters accurately to ensure timely claims for payment.
  • Supervise, coach and mentor direct care staff to enhance their skills and performance.
  • Ensure effective communication and coordination between services, staff and stakeholders to support clients.
  • Assist clients in managing their finances according to our policies and procedures.
  • Embody the CPL Way by understanding each person, providing great service, working together and finding better ways.

Selection Criteria:

The ideal candidate will possess experience in a similar role within the Aged Care sector, relevant tertiary qualifications and a strong commitment to delivering exceptional customer service. The successful applicant will also demonstrate a proactive approach, excellent interpersonal and communication skills, and the ability to work autonomously and collaboratively within teams.
