Key Responsibilities- Disease and Treatment Related Care:
- Initial assessment of need using a systematic evidence-based approach
- Maintaining accurate records of nursing assessments and outcomes
- Triage patients to determine areas of need and levels of assistance required
- Maintaining understanding of the rationale for the use of treatments and therapies used in the management of prostate cancer and their related clinical effects
- Identifying opportunities for patients to participate in clinical trials
- Providing ongoing psychosocial support in accordance with individual needs
- Coordinated Care:
- Coordinates patient caseloads and ensures continuity of care
- Ensures key elements of care occur in an ordered and timely manner
- Identifies and makes referrals to appropriate health and social agencies as needed
- Information Provision and Education:
- Identifying educational and supportive care needs of people affected by cancer
- Providing direct education to people affected by cancer within various settings
- Supporting and empowering patients to influence decisions concerning their care
- Collaborative and Therapeutic Practice:
- Maintaining communication and links with other members of the cancer care team
- Supporting the delivery of multidisciplinary care and participating in MDT meetings or video conferencing

Requirements- 5+ years' post-graduate experience in full-time equivalent in prostate cancer nursing field
