This role offers an exciting opportunity to contribute to a dynamic and progressive multidisciplinary team in the Northern Rivers. As a Community Mental Health Case Manager, you will be part of a supportive team delivering person-centered care within a time-limited framework.
About the Role:Delivering individual and group-based treatment, managing clinical loads, and collaborating with a multidisciplinary treatment process are key responsibilities of this position. You will provide assessments, evidence-based treatment, and case management services to adults experiencing psychiatric illnesses, adhering to principles of Trauma Informed Care and Recovery.
Key Responsibilities:- Provide assessments, evidence-based treatment, and case management services to adults 18 years and over referred to the service.
- Deliver individual and group-based treatment and manage individual clinical loads.
- Collaborate as part of a multidisciplinary treatment process.
To succeed in this role, you must have strong communication and interpersonal skills, enabling effective collaboration with people from diverse backgrounds and cultures. Excellent problem-solving and analytical skills are also essential, along with the ability to think critically and creatively in complex situations.
