Care Manager Lead Opportunity

Ozcare is seeking a highly skilled and experienced Care Manager Lead to join our team. As a key member of our Care Manager team, you will play a pivotal role in developing and growing our Home Care Package business within our branch.

Your Role

As a Care Manager Lead, you will be responsible for providing leadership to a dedicated Care Manager team in the planning, managing, and leading client care and support across a range of programs. You will also play a key role in coordinating the involvement of Ozcare's multi-disciplinary teams and other services to support effective care.

Key Responsibilities
  • Manage and coordinate care services to ensure the highest standards of care
  • Develop and implement care plans tailored to individual client needs
  • Liaise with clients, families, and healthcare professionals to ensure comprehensive care
  • Foster strong relationships with clients, stakeholders, and team members, while assisting clients in meeting their unique needs and goals
  • Ensure compliance with all relevant legislation and organizational policies
  • Monitor and evaluate care services to continuously improve quality and efficiency
  • Monitor the budget of Home Care Packages and ensure clients understand their funding and fees
  • Work closely with the Branch Manager and Care Managers to ensure service growth
  • Provide supervision and support to the Care Manager team

Requirements

To be successful in this role, you'll need to demonstrate:

  • Experience in case management and client care coordination in a Community Aged Care setting
  • Strong verbal, written, and interpersonal communication skills to communicate and work effectively with a broad range of people
  • Ability to develop staff, foster teamwork, and effectively work as a team member while providing leadership
  • Proof of immunity or willingness to become vaccinated against Measles, Mumps, Rubella (MMR), Varicella (Chicken Pox), and Pertussis (Whooping Cough)
  • Current Police Screening Certificate
  • Current First Aid and CPR Certificate
  • Current NDIS Worker Screening Check (Yellow Card) and Blue Card (Working with Children Check)
  • Current National Police Check
  • Current Queensland driver's licence
  • Tertiary qualifications and/or relevant experience in a health-related field
