Job Title: Advanced Healthcare Practitioner

Key Responsibilities:

  • Developing comprehensive care plans and conducting thorough assessments
  • Crafting government-compliant care management plans that drive holistic outcomes
  • Coordinating personal care supports and facilitating quality assurance initiatives
  • Operating the care planning and management systems efficiently

Required Qualifications:

  • Current registration with AHPRA as a Registered Nurse
  • Substantial community nursing experience is essential
  • A satisfactory Australian National Police check
  • A valid Working With Children's Check
  • Current first aid and CPR certification
  • A driver's licence and access to a reliable vehicle

Preferred Skills:

  • Excellent time management and prioritisation skills
  • Proven ability to develop strong relationships with clients and colleagues
  • Intermediate to Advanced Microsoft Office skills, particularly in Excel, Word, and Outlook
