Job Description

We are seeking a skilled professional to join our multidisciplinary team at Northern NSW Local Health District. The successful candidate will provide comprehensive and appropriate assessment and intervention services as part of a healthcare team for patients/clients in the region.

The ideal candidate will have excellent communication and interpersonal skills, with the ability to build rapport with patients/clients from diverse backgrounds. They will also possess strong problem-solving skills, with the ability to navigate complex healthcare systems and advocate for patient needs.

About the Role
  • Provide social work assessment and intervention services to patients/clients in accordance with their needs and goals.
  • Work collaboratively with other healthcare professionals to ensure comprehensive care for patients/clients.
  • Develop and implement individualized plans to address social determinants of health and improve patient outcomes.
Required Skills and Qualifications
