Job Opportunity: Care Coordinator

The Northern Health organization is seeking a highly skilled and experienced care coordinator to join their Hospital Admission Risk Program (HARP) team in a fixed term position.

About the Role

This role requires a registered nurse with minimum 5 years clinical experience and excellent knowledge in care coordination or case management. The successful candidate will be responsible for delivering safe, high-quality care to clients with complex needs and/or chronic disease who are at risk of readmission to hospital.

  • Minimum 5 years clinical experience
  • Experience and excellent knowledge in care coordination or case management
  • Clinical leadership in the continuing care setting
  • Demonstrated advanced clinical assessment, intervention planning, and delivery skills
  • Demonstrated high level of knowledge of community, residential, and acute/subacute services that support clients with chronic disease and complex needs
  • Demonstrated high level problem-solving and planning ability
