The Social Work Service at Monash Health assists patients and families with psychosocial support and advocacy through assessment, counseling, information provision, access to community resources, and discharge planning. Our team works closely with internal and external stakeholders to provide care founded on current evidence-based practice for patients accessing care through the inpatient and community-based setting.
About YouWe are looking for a skilled and experienced social worker who is able to work collaboratively with the broader multidisciplinary team and possess excellent written and verbal communication skills. You will have a sound understanding and working knowledge of the range of service systems, including Family Violence, Child Protection, My Aged Care, Transition Care Program, and the National Disability Insurance Scheme (NDIS), Mental Health and the recovery model.
You will be responsible for carrying a complex clinical caseload, providing team leadership, leading Quality and Improvement initiatives, and providing education and training to staff and students. You will also be required to participate in the Emergency Department and on-call rosters.
