Disability Support Worker Role South East/berwick

**Full Job Description**
Our role as an NDIS support worker is to assist people with disabilities in their daily lives.
To access services and supports to help achieve goals and improve quality of life.
Experience working with eating disorders, palliative care, mental health and physical mobility will be preferred.
You duties may include:
Hospital: Care within a hospital and home home settings.
Domestic inclusion: Helping with household tasks such as cleaning, cooking and laundry.
Community access: Supporting individuals to participate in community activities such as social outings, shopping, and attending appointments.
Behavioural support: Providing support and strategies to manage behaviours.
Emotional support: Providing emotional support and guidance to individuals with disabilities and their families.
Health and well being: Monitoring and assisting with the management of health and well being needs such as medication management, exercise, and nutrition.
As an NDIS disability support worker, you will work closely with individuals with disabilities, their families, and other healthcare professionals to develop and implement individualised care plans that meet their specific needs and goals. You will also need to maintain accurate records of care provided.

Relevant experience and qualification required as we are a registered NDIS provider.
