Role Overview

As a Care Coordination Specialist, you'll be instrumental in delivering high-quality care services, consulting with clients, caregivers, service providers, and other key stakeholders.

You'll develop personalized care plans, ensuring seamless support and empowerment for individuals with complex needs.

Requirements

To succeed in this role, you'll need:

  • Tertiary qualifications in relevant health or social sciences
  • Demonstrated commitment to Annecto's Purpose and Principles
  • Previous experience in case management or a similar field
  • Excellent communication and interpersonal skills
  • Ability to work autonomously and efficiently

Great to Have

A desirable asset for this role would be:

  • Knowledge of local community resources and services
  • Experience working in a similar capacity in a not-for-profit organisation

Eligibility
