Job Title: Home Care Package Care Partner

Are you passionate about delivering high-quality care services to individuals in their homes? We are seeking a skilled and dedicated Home Care Package Care Partner to join our team.

The successful candidate will be responsible for assisting in individualized services delivered to customers, performing assessments, managing participant budgets, and communicating with stakeholders.


Key Responsibilities:
  • Undertake assessments of participants in their homes to identify their needs and develop personalized care plans.
  • Ensure follow-up actions are taken to reduce risks, refer allied health professionals, and order necessary equipment.
  • Provide ongoing case management support to participants, focusing on their clinical and holistic needs.
  • Monitor and communicate on participants' wellbeing, reporting progress on care assessments, care plans, and healthcare plans.
  • Document participants' goals, progress, and outcomes using person-centered, wellness, reablement, and enabling strategies.
  • Perform extensive administrative duties, including data entry, processing invoices, and following up participant enquiries.
  • Liaise with relevant community-based agencies and service providers to ensure integrated and cohesive service delivery.
  • Assist in participant service schedules, including service planning, confirmation, and short-notice placements or cancellations.
  • Work collaboratively with participants to manage their budgets and monitor their financial situation.
  • Communicate effectively with key stakeholders, both internally and externally.

Requirements:
  • Relevant qualifications and experience in a care-related field.
  • Excellent communication and interpersonal skills.
  • Able to work independently and as part of a team.
  • Ability to prioritize tasks and manage time effectively.
  • Basic computer skills and familiarity with software applications.
