We are seeking a skilled Care Manager to join our team at Ozcare. As a Care Manager, you will play a key role in planning, managing, and leading client care and support across a range of programs.
Key Responsibilities- Manage and coordinate care services to ensure the highest standards of care.
- Develop and implement care plans tailored to individual client needs.
- Liaise with clients, families, and healthcare professionals to ensure comprehensive care.
- Foster strong relationships with clients, stakeholders, and team members, while assisting clients in meeting their unique needs and goals.
- Ensure compliance with all relevant legislation and organizational policies.
- Monitor and evaluate care services to continuously improve quality and efficiency.
- Monitor the budget of Home Care Packages and ensure clients understand their funding and fees.
- Optional salary packaging, meal entertainment cards, and novated leases.
- Private health insurance discounts.
- Uniform and laundry allowance.
- Employee Assistance Program.
- Demonstrate experience in case management and client care coordination in a Community Aged Care setting.
- Demonstrated strong verbal, written, and interpersonal communication skills to communicate and work effectively with a broad range of people.
- Proof of immunity, or willingness to become vaccinated against, Measles, Mumps, Rubella (MMR), Varicella (Chicken Pox), and Pertussis (Whooping Cough).
- Current Police Screening Certificate.
- Current First Aid and CPR Certificate.
- Current NDIS Worker Screening Check (Yellow Card) and Blue Card (Working with Children Check).
- Current Queensland driver's licence.
- Tertiary Qualifications and/or relevant experience in a health-related field (e.g. Health and Human services).
