Care Coordinator Leader

This is an exciting opportunity to work in a brand-new facility, designed with the future of aged care in mind.
The Care Coordinator Leader will lead our Meaningful Life program, developing opportunities for meaningful engagement, purposeful participation, social events and activities for all residents.
Key responsibilities involve driving initiatives that support residents' physical, emotional and spiritual wellbeing and overall quality of life.
The role involves developing resident care plans, completing key lifestyle assessments, and working together with clinical teams and allied health professionals to provide a tailored approach for each resident.
We are looking for someone who can coach care team members to deliver meaningful engagements between like-minded residents, liaise with residents to develop activities and engagements that meet their cultural, spiritual, social and physical needs and preferences, and engage with residents' families to encourage visitation and socialisation.

In this role you will be responsible for:
- Developing and implementing care plans that cater to individual residents' needs;
- Coordinating activities and events that promote socialisation and engagement;
- Liaising with residents, families and healthcare professionals to ensure high-quality care;
- Providing guidance and coaching to care team members to enhance their skills and knowledge;
