Personalized Care Officer

 

 

About the Role
  • Develop personalized care plans that cater to residents' preferences, abilities, and needs.
  • Record and maintain care plans, progress notes, activity attendance, and other relevant information in a timely manner.
  • Organize and facilitate engaging activities and outings that meet the diverse interests and needs of residents.
  • Ensure resident safety and well-being during outings, responsible for medication management.
  • Supervise and support volunteers during activities and outings.
  • Prepare and distribute the monthly newsletter to residents and families.
  • Participate in internal audits and strive for continuous improvement.
  • Engage in training programs to improve professional skills.
Qualifications & Experience
  • Minimum 3 years' experience in Aged Care, specifically with dementia care.
  • Certificate IV in Leisure & Lifestyle and/or Certificate IV in Aged Care.
  • Dutch speaking preferred, additional secondary languages an advantage.
  • Current First Aid Certificate or willingness to acquire.
  • National Police Check renewable every three years or willingness to acquire.
  • Competent in computer use, preferably with knowledge of Publisher and PowerPoint.
