The Role:We seek an experienced CDM (Chronic Disease Management) Nurse, Enrolled or Registered, to join our team at a Rockhampton Medical Centre.
This position will support GPs and their patients in identifying and managing chronic conditions, aiming to prevent hospital admissions.
The role is preferably full-time but part-time options can be discussed with the right candidate.
Key Responsibilities:- Prepare Care Plans and Team Care Arrangements to assist patients in managing their diseases
- Empower patients to take control of their health
- Coach patients on healthy lifestyle principles
- Enable patients to access ongoing care from a multidisciplinary team
- Assist in early identification of chronic disease through regular health assessments
- Collaborate with GPs to promote preventative healthcare strategies
- Increase the number of patients receiving a care plan for the first time
- Ensure regular review of patient care plans
- Manage administrative tasks related to CDM appointments
