This role involves working with GPs and patients to identify and manage chronic conditions, focusing on keeping patients out of hospital.
About the RoleWe are seeking an experienced CDM Nurse, Enrolled or Registered, to join our team in Rockhampton. The successful candidate will be involved in tasks such as preparing care plans and team care arrangements, empowering patients to take ownership of their health, coaching patients on health behavior change principles, enabling patients to access ongoing care from a multidisciplinary team, assisting in early identification of chronic disease through health assessments, working with GPs to promote preventative healthcare, increasing the number of patients receiving a care plan for the first time, ensuring regular review of patient care plans, and managing administrative tasks related to CDM appointments.
The Ideal CandidateWe're looking for nurses passionate about patient care and collaborative teamwork. You will bring AHPRA registration as an Enrolled or Registered Nurse, experience in Chronic Disease Management and health assessments, knowledge of Health Assessment (highly regarded), strong organizational and time management skills, excellent communication and customer service skills, and a personable approach and passion for people.
