At the heart of our organisation, we're seeking skilled professionals to take on the challenge of delivering exceptional community-based nursing care.
Key Responsibilities:- To contribute to assessments, early detection and prevention of health issues in clients.
- Work collaboratively with a registered nurse, exercising autonomy while upholding high standards of practice.
- Maintain effective communication with stakeholders to ensure seamless delivery of care.
- Contribute to care plans and case conferencing.
For this role, you'll need:
- A relevant qualification and AHPRA registration.
- Substantial experience in acute settings.
- A passion for providing holistic care that enhances clients' independence and quality of life.
- Flexibility to work various shifts, including mornings and evenings (approx. 70 hours per fortnight).
- Self-motivation and an autonomous working style.
- A current police check and valid Victorian driver's license with own vehicle.
