A fulfilling part-time position (4 days a week) based in Elderslie offers the opportunity to empower tenants at BaptistCare's Narralling Community to live life on their own terms.

About the Role

This is a chance to make a meaningful difference in the lives of seniors, Aboriginal and Torres Strait Islander peoples, and single-parent families. As a Personalised Support Coordinator, you will provide effective assessment and planning to coordinate and support tenants and household members access to a range of individualised services to improve their wellbeing and achieve their goals in life.

Key Responsibilities
  • Assess and plan individualised services to meet the needs of tenants and household members.
  • Collaborate with cross-functional teams to deliver high-quality support services.
  • Foster positive relationships with tenants, family members, and community stakeholders.
  • Develop and maintain accurate records of tenant interactions and service delivery.
Required Skills and Qualifications

To be successful in this role, you will require:

  • A Certificate IV – Social Work or Community Services qualification.
  • Experience in social housing, community services, or a related discipline.
  • Demonstrated ability to facilitate effective support services that achieve positive outcomes for tenants and household members.
  • High-level negotiation skills and ability to liaise with key stakeholders.
  • Competence in service assessment and documentation.
