Job Description:

We are seeking a dedicated professional to fill the role of Case Manager. The successful candidate will be responsible for assisting in the planning, implementation and evaluation of consumer care plans.

As a key member of our team, you will work closely with direct care employees to provide leadership and support, utilizing strong critical thinking and decision-making skills.

The ideal candidate will have sound knowledge of aged care, OHS, Home and Community Care and other relevant legislation, as well as excellent communication skills.

A National Police Check and valid Victorian Driver's Licence are required for this position.

Key Responsibilities:
  • Assist in the planning, implementation and evaluation of consumer care plans.
  • Provide leadership and support to direct care employees.
  • Utilize strong critical thinking and decision-making skills.
  • Maintain up-to-date knowledge of regulations and legislation.
