Seeking a skilled Registered Nurse to join our team in delivering compassionate support, education, and personalised care planning for individuals living with dementia and their carers.

The successful candidate will work in a dynamic community setting, visiting clients at home, clinics, and support groups, building strong relationships across multiple locations on the Central Coast.

  • Key Responsibilities

  • Provide individualised care, support, and education to clients and families.

  • Deliver memory screenings and ongoing support tailored to each client's needs.

  • Collaborate with other healthcare professionals, carers, and community groups to provide comprehensive, culturally sensitive care.

  • Develop and implement effective care plans and participate in dementia care planning.

  • Travel across Central Coast locations; use fleet cars as needed.

Requirements

">

  • Registered Nurse with current registration with AHPRA.

  • Experience in dementia care, community nursing, aged care, or dementia services preferred.

  • Skilled in person-centred assessment, care planning, and intervention.

  • Excellent communication and interpersonal skills, with the ability to build trust with clients and families.

  • Valid NSW Driver's Licence.
