About the Role

As a key member of the Community Chronic and Complex Care Directorate, the Dementia & Behaviour Support Services (DABSS) team collaborates with people living with dementia and their carers to deliver compassionate support, education, and personalized care planning.

Responsibilities:
  • Deliver customized care, support, and education to clients and families.
  • Conduct memory screenings and provide ongoing support tailored to individual needs.
  • Collaborate with other healthcare professionals, carers, and community groups to offer well-rounded, culturally sensitive care.
  • Develop care plans and participate in dementia care planning.
  • Travel across Central Coast locations using fleet cars as needed.
About You / Qualifications:
  • Registered Nurse with current AHPRA registration.
  • Experience in dementia care, community nursing, aged care, or dementia services is preferred.
  • Skilled in person-centred assessment, care planning, and intervention.
  • Excellent communication skills with the ability to build trust with clients and families; organized and committed to learning.
  • Valid NSW Driver's Licence.
