Compassionate Dementia Support Nurse

We are seeking a compassionate Registered Nurse to provide personalized support, education, and care planning for individuals living with dementia.
The successful candidate will have professional independence in their practice, supported by an experienced multidisciplinary team. They will work closely with other health professionals, carers, and community groups to provide well-rounded and culturally sensitive support.
As a key member of the Community Chronic and Complex Care Directorate, you will enjoy a rewarding mix of community-based care, memory screenings, and ongoing support tailored to each person's individual needs. With flexibility to work across multiple locations on the Central Coast, your week will be varied.
This role involves working closely with people living with dementia and their carers to provide compassionate support, education, and personalised care planning. The successful candidate will have the opportunity to develop their skills and expertise in this area.
