Job Overview

This role focuses on delivering high-quality primary health care services to children aged 0-5 years and their families in a dynamic and supportive environment.


The successful candidate will be responsible for providing comprehensive assessments, identifying family strengths and vulnerabilities, and implementing evidence-based interventions to promote optimal health outcomes.

Key Responsibilities:
  • Conduct thorough physical, emotional, and psychosocial assessments of children and their families.
  • Develop and implement individualized care plans to address identified needs and promote healthy development.
  • Collaborate with multidisciplinary teams to provide seamless care coordination and support.
  • Maintain accurate and detailed records of client interactions and progress.

Requirements:
  • Demonstrated expertise in child and family health nursing or midwifery with a strong understanding of primary health care principles.
  • Current registration as a Registered Nurse and/or Midwife with the Australian Health Practitioner Regulation Agency (AHPRA).
  • Specific postgraduate qualification in Child and Family Health Nursing or near completion.
  • Proven ability to work effectively in partnership with families and other healthcare professionals.
