Job Opportunity: Client Integration Specialist

We are seeking a highly skilled and experienced professional to fill the role of Client Integration Specialist. This position is crucial in ensuring the seamless integration of clients into our programs and services.

The successful candidate will work closely with clients, their families, and internal teams to develop personalized integration plans and coordinate support services tailored to individual needs.

Key Responsibilities:

  • Develop and implement customized integration plans for clients, guaranteeing their participation in our programs and services is smooth and tailored to their requirements.
  • Collaborate with internal teams and external stakeholders to coordinate support services and resources for clients.
  • Conduct assessments to identify client needs and preferences, and provide recommendations for program adjustments or enhancements.
  • Offer guidance and support to clients and their families throughout the integration process, addressing any concerns or challenges that may arise.
  • Monitor client progress and outcomes, making adjustments to integration plans as needed to ensure positive results.

Required Skills and Qualifications:

  • Bachelor's degree in social work, psychology, disability studies, or a related field.
  • Minimum of 3 years of experience working in a similar role, preferably in the disability support sector.
  • Strong interpersonal and communication skills, with the ability to build rapport and trust with clients and their families.
  • Knowledge of the NDIS framework and relevant legislation governing disability support services in Australia.
  • Ability to work collaboratively in a multidisciplinary team environment.
