About the Role

Ozcare is seeking a highly skilled and experienced Care Manager Lead to join their team. As a key member of the Care Manager team, you will play a vital role in developing and growing the Home Care Package business within the branch.

Key Responsibilities
  • Provide leadership to the Care Manager team in planning, managing, and leading client care and support across various programs.
  • Coordinate the involvement of Ozcare's multi-disciplinary teams and other services to ensure effective care.
  • Ensure consistency in governance and compliance while driving service growth.
  • Develop and implement care plans tailored to individual client needs.
  • Liaise with clients, families, and healthcare professionals to ensure comprehensive care.
  • Foster strong relationships with clients, stakeholders, and team members.
  • Monitor and evaluate care services to continuously improve quality and efficiency.
  • Manage the budget of Home Care Packages and ensure clients understand their funding and fees.
  • Work closely with the Branch Manager and Care Managers to ensure service growth.
  • Provide supervision and support to the Care Manager team.

Requirements
  • Demonstrate experience in case management and client care coordination in a Community Aged Care setting.
  • Demonstrate strong verbal, written, and interpersonal communication skills.
  • Show the ability to develop staff, foster teamwork, and effectively work as a team member while providing leadership.
  • Provide proof of immunity or willingness to become vaccinated against Measles, Mumps, Rubella, Varicella, and Pertussis.
  • Hold a current Police Screening Certificate.
  • Provide a current First Aid and CPR Certificate.
  • Provide a current NDIS Worker Screening Check (Yellow Card) and Blue Card (Working with Children Check).
  • Hold a current Queensland driver's licence.
  • Provide any tertiary qualifications and/or relevant experience in a health-related field.
