Care Manager Lead Role

Ozcare is seeking a highly skilled and experienced Care Manager Lead to join their team. As a key member of the Care Manager team, you will be responsible for developing and growing the Home Care Package business within the branch.

Your Key Responsibilities

Manage and coordinate care services to ensure the highest standards of care
Develop and implement care plans tailored to individual client needs
Liaise with clients, families, and healthcare professionals to ensure comprehensive care
Foster strong relationships with clients, stakeholders, and team members, while assisting clients in meeting their unique needs and goals
Ensure compliance with all relevant legislation and organizational policies
Monitor and evaluate care services to continuously improve quality and efficiency
Monitor the budget of Home Care Packages and ensure clients understand their funding and fees
Work closely with the Branch Manager and Care Managers to ensure service growth
Provide supervision and support to the Care Manager team

Requirements

To be successful in this role, you'll need to demonstrate:
Experience in case management and client care coordination in a Community Aged Care setting
Strong verbal, written, and interpersonal communication skills to communicate and work effectively with a broad range of people
Ability to develop staff, foster teamwork, and effectively work as a team member while providing leadership
Proof of immunity or willingness to become vaccinated against Measles, Mumps, Rubella (MMR), Varicella (Chicken Pox), and Pertussis (Whooping Cough)
Current First Aid and CPR Certificate
Current NDIS Worker Screening Check (Yellow Card) and Blue Card (Working with Children Check) - or apply for these prior to commencing in the role
Current National Police Check, prior to commencement
Hold a current Queensland driver's licence
Provide any tertiary qualifications and/or relevant experience in a health-related field (e.g. Health and Human services)
