We are seeking a dynamic and experienced professional to join our team as a Client Care Manager.
The role involves managing both aged and disability clients, working one on one with them and their support network to develop customised care plans. The ideal candidate will have consolidated nursing skills and knowledge in intellectual disabilities, stroke, dementia, diabetes, paraplegia, complex bowel management, wound care and catheter care.
The Client Care Manager will oversee a team of nurses and caregivers to deliver a high standard of clinical care, maintaining meticulous records and documentation accurately and thoroughly. They will also be responsible for representing and promoting the organisation's brand in all aspects of their work.
This is an exciting opportunity for a self-led leader who can take responsibility for their own behaviour and success. We are looking for someone with exceptional organisation, planning, time management and willingness to embrace advice. They should also be able to demonstrate ability in maintaining high clinical standards and report client care concerns to the multidisciplinary team.
The ideal candidate will be a positive and dynamic individual who thrives in a fast-paced environment, but also has the patience to slow down and spend the time required to make a real difference in clients' lives.
Mandatory Requirements:
- A Registered Nurse with AHPRA registration and no conditions or limitations imposed on practice, minimum 5 years experience.
- Experience in either a Home Care setting or Aged Care facility highly regarded.
- CPR & First Aid Certification.
- Current Drivers Licence.
- National Police Check.
- Working With Vulnerable People Check - employee status, or willing to obtain.
- NDIS Worker Screening Check, or willing to obtain clearance.
- NDIS Worker Orientation Module Completion.
