Community Care Nurse

As a compassionate and respectful Enrolled Nurse, you will provide clinical in-home services to support independent living in the community.
Key Responsibilities:- Deliver high-quality clinical care including care planning, medication management, wound care, chronic disease management, palliative care, and diabetes management.
- Supervise non-clinical team members in the community.
- Manage intake, care planning, and reporting processes.
- Conduct client assessments with a focus on Consumer Directed Care.
- Coordinate quality, person-centered care tailored to client needs.
- Qualified Enrolled Nurse.
- Current AHPRA registration.
- First Aid & CPR certification.
- Australian Police Check.
- Professional Indemnity Insurance.
- Experience in community care assessment and planning.
- Valid driver's license and reliable vehicle.
- Smart mobile device (iPhone/Android).
- Passion for high-quality, client-focused community care.
- Cert IV in Training and Assessment (highly desirable).
- Strong communication skills.
- Ability to work independently and in teams.
- Excellent time management and organizational skills.
- Commitment to ongoing professional development.
- Residing in Western Australia is required.
