Job Overview

We are seeking a dedicated and compassionate individual to join our outreach team as an Outreach Support Worker.

Key Responsibilities
  • Lead and support outreach activities that promote social connection and engagement.
  • Assist in preparing and serving meals, ensuring a welcoming dining experience for clients.
  • Deliver exceptional service with a strong focus on enhancing the client experience.
  • Follow individual care plans to meet each client's unique needs and preferences.
  • Provide mobility assistance to support safe and comfortable movement.
  • Stay attuned to emotional and physical needs, ensuring care is person-centred and responsive.

Requirements
  • A genuine passion for supporting people with disability or the frail aged.
  • Strong communication skills—both written and verbal.
  • Confidence using systems and technology in daily work.
  • NDIS Worker Screening Check (or willingness to obtain).
Desirable Qualifications
  • Certificate III in Individual Support, Disability, Aged Care, or a related field.
  • Current First Aid certification.
  • Experience working in centre-based settings or with people with disability.
