Job Description:

We are seeking dedicated and compassionate individuals to fill the role of Personal Care Specialist. As a Personal Care Specialist, you will provide comprehensive support services to residents in their home, extending to the community and health planning.

You will assist residents with activities of daily living, including medication management, personal care, food preparation, and mealtime assistance. You will also plan, conduct, and follow up on resident health appointments and maintain communication with team members.

Establishing and maintaining effective links and positive working relationships with residents, families, and stakeholders is crucial. Creating an empowering environment for people with disabilities and developing resident-directed care programs to deliver individual goals and outcomes are key responsibilities.

Contribution to meal preparation, transportation of residents using vehicles, participation in monthly Care Team Meetings, and supervision sessions are also expected. Completing house routines and cleaning to ensure a safe, well-presented, and clean home for residents is essential.

Shift work across various hours and shifts, including morning, day, evening, sleepover, weekday, and weekend work, is required according to a specific roster line.

The Role Includes:
