Key Responsibilities:

  • Actively liaise with community service providers and coordinate care to support consumer engagement with community and discharge planning", "
  • Contribute to ongoing evaluation and improvement initiatives and portfolio work, in collaboration with leadership", "

Requirements:

  • Excellent communication skills", "
  • Demonstrated clinical experience in relation to assessment, care planning and provision of clinical interventions", "
  • Knowledge and experience in consumer centred care; supported decision making; and the recovery model", "
  • Sound knowledge of the mental health interface and experience working with NDIS providers", "
  • A keen interest in working in a specialist neuropsychiatry unit", "
