Job Overview:

The position of a Grade 1 social worker is available in our hospital setting.

This role involves providing patient-centered care and collaborating with a team to deliver a holistic approach to service delivery.

Key Responsibilities:
  • Conduct comprehensive psychosocial assessments and develop individualized care plans.
  • Work closely with internal and external stakeholders to provide evidence-based practice for patients accessing care through inpatient and community settings.
  • Assist with complex discharges and bed-based service delivery, including rosters servicing the Emergency Department and experience in acute and women's and children's wards.
Requirements:
  • AASW accreditation Bachelor degree or entry-level Master's Degree of Social Work.
  • Excellent organizational skills and a positive attitude towards work.
  • Experience working with diverse communities and ability to work autonomously.
  • Well-developed clinical assessment and discharge planning skills.
  • Current Victorian driver's license.
