**Job Title:** Team Leader Heidelberg Acas

**Job Description:

  • This is a leadership role where you will be responsible for overseeing the day-to-day operations of the Heidelberg Aged Care Assessment team.
  • You will work closely with the Community Aged Care Services Manager to ensure that the program is delivered effectively and efficiently.

**Responsibilities:

  • Provide operational oversight and support to the Heidelberg Aged Care Assessent Team.
  • School stakeholders to support internal relationships, organisational bed flow and community connections.

**Requirements:

  • Well developed leadership skills.
  • A passion for Aged Care Services.
  • Strong commitment to delivering high quality assessment outcomes.
