Job Opportunity

We are seeking a skilled Healthcare Professional to join our team as a Nurse Consultant. The successful candidate will be responsible for providing clinical support to our service team, optimizing health outcomes for NDIS participants and their families.

The role involves working closely with our team to support assessments and provide recommendations on services and plans. You will also be responsible for developing and implementing individualized care plans in collaboration with clients, families, and healthcare professionals.

Key Responsibilities:

  • Provide basic clinical support to the service team
  • Work with the service team to support assessments and provide recommendations on services and plans
  • Develop and implement individualized care plans in collaboration with clients, families, and healthcare professionals
  • Maintain accurate and comprehensive records of client information, progress, and outcomes

Requirements:

  • NDIS Worker Check (or willing to obtain)
  • Currently registered with AHPRA
  • Minimum of one year of supervised post-registration practice
  • Must have the right to work in Australia
  • Current driver's license and a willingness to drive to support client needs
  • Flexibility and a willingness to travel and work at all locations
