About the Role

We are seeking a dedicated and compassionate Support Worker to join our team at Identitywa. As a Support Worker, you will play a pivotal role in enhancing the quality of life for individuals with disabilities.

Key Responsibilities
  • Provide person-centered support to participants, assisting them in achieving their personal goals and promoting community inclusion and social engagement.
  • Assist participants with daily living skills and activities within the home, as well as accompanying them on community outings for recreational activities, appointments, and religious events.
  • Promote independence and self-advocacy among participants, while also providing emotional support and guidance when needed.
  • Collaborate with the multidisciplinary team to develop and implement individualized support plans, ensuring that participants' needs are met and their rights are respected.
  • Contribute to a positive and inclusive work environment, fostering a culture of respect, empathy, and understanding among colleagues and participants.
Requirements
  • Minimum 2 years of experience in the disability sector, with a strong understanding of person-centered support principles and practices.
  • Experience working with individuals who have behaviors of concern, as well as a strong understanding of behavior support plans and strategies.
  • Passion for working with people in their homes and the community, with a commitment to promoting independence and self-advocacy.
  • Ability to work autonomously and as part of a team, with excellent communication and interpersonal skills.
  • Current NDIS Worker Screening Check, Provide First Aid Certificate, and a valid driver's license (Green P plates minimum).
