Elderly Care Specialist

We are seeking a compassionate and skilled nurse with a strong interest in mental health to join our team as a Community Nurse.
This is a meaningful opportunity to make a real difference in the lives of older people—helping them stay well, stay safe, and stay connected.
The successful candidate will draw on primary health care principles to deliver holistic, person-centred care to older adults experiencing mental health challenges, chronic conditions, and complex needs.
Key Responsibilities:- Providing direct clinical nursing care to clients in their homes in line with best practice nursing procedures
- Conducting clinical assessments, dressings, medication assessment and catheter changing
- Developing a holistic nursing care plan in collaboration with client, carer, Care Advisor and Allied health where relevant
- Providing safe, quality nursing practice to clients with specific needs and/or complex needs eg. Wound care, palliative care, education and support
- Liaising with all parties involved in the care of the client to maintain holistic health care and wellbeing
- Ensuring all documentation is accurate and completed in a professional and timely manner
- A genuine passion for providing high-quality care to the elderly
- Current AHPRA registration following completion of a Bachelor of Nursing or similar
- Minimum 3 years post-graduate nursing experience
- Experience in clinical assessments
- Initiative and the ability to work independently
- Strong computer skills especially Microsoft Office products and ability to pick up new systems
