This role is a vital part of our Out of Hospital Care program, aiming to enhance the quality of life for our clients while enabling them to live independently.

  • Domestic Support (encompassing cleaning, laundry, transportation, shopping, and meal preparation)
  • Hygiene Assistance (including supervising or assisting with daily bathing or showering routines)
  • Mobility Support Services
  • Client Plan Management (including documentation and progress note keeping)
  • Effective Communication and Liaison (representing the team through professional interactions with clients, families, and relevant professionals)
  • Respectful Interactions and Dignity Preservation
  • Shifts will be completed between 7:00 am and 7:00 pm, Monday to Friday, without overnight stays, evening shifts, or weekends.

Key Responsibilities:

  • Provide domestic assistance, including cleaning, laundry, transportation, shopping, and preparing meals.
  • Assist with daily hygiene tasks, such as bathing or showering.
  • Offer mobility support to clients.
  • Manage client plans, including documentation and progress notes.
  • Represent the team professionally and effectively communicate with clients, families, and relevant professionals.
  • Preserve the dignity and respect the privacy of clients during interactions.
