Job Description
  • We are seeking a highly skilled and experienced Social Worker to join our team. The successful candidate will provide medical focused social work services to patients in a hospital inpatient setting.
  • The ideal candidate will have a strong understanding of the range of issues affecting people of Aboriginal and Torres Strait Islanders, as well as experience working with multidisciplinary teams to assist in the discharge of patients with complex social issues.

Key Accountabilities:

  • Provide medical focused social work in a Hospital inpatient setting.
  • Work with the multidisciplinary team to assist in the discharge of patients with complex social issues.
  • Assist in the development of a discharge plan consistent with the patient's assessment by members of the patient's treatment team, the patient and the patient's carer.

Selection Criteria:

  • Considerable Hospital experience working as a Social Worker.
  • Eligible for registration with the Australian Association of Social Workers.
  • Strong knowledge of the Aged Care sector.
