Care Coordinator

The key objective of this position is to provide patient care as directed by the Nurse Manager, adhering to the nursing care plan and under the supervision of the Registered Nurse/Midwife.
Main duties include effective communication with the team, patients, and families, accurate health information & documentation for patients, and contributing to a person-centred approach to nursing.
This role requires registration as an Enrolled Nurse (Division 2) with Australian Health Practitioner Regulation Agency (AHPRA), demonstrated skill in Aged Care Nursing, and flexibility to work within a roster including day, evening and night shifts.
As a member of the multi-disciplinary healthcare team, the successful candidate will demonstrate understanding of legislation, policies, and scope of practice for an Enrolled Nurse.
